Option 1
Download first then fill it out (Recommended)

you can save the PDF to your computer before you fill it out and fill it out on your local computer. With this option, make sure you resave the file when you are done filling everything out and send it back over to me!

Option 2:
Fill this out online

If you fill this out online, when you have completed it, make sure you hit the PRINT button at the top right (NOT the download PDF button — if you select download, it will just download the blank PDF) and change the destination to “Save as PDF” instead of a printer.
